A partial hospitalization program (PHP), also called day treatment, is the most intensive level of outpatient care. You spend most of the day in therapy, groups and medical check-ins on most weekdays, then go home or to sober housing at night.

PHP at a Glance

  • What it is. Full-day clinical treatment on most weekdays. No overnight stay.
  • Time commitment. At least 20 hours a week. Network PHP programs typically run about 5 hours a day, 5 or 6 days a week.
  • Program length. Set by your treatment plan. Many people step down from PHP to IOP, around 90 days across the two.

What Is a Partial Hospitalization Program?

PHP sits between intensive outpatient care and residential treatment. It gives you the structure and clinical attention of a full treatment day without living at a facility.

The ASAM Criteria, the framework providers and insurers use to match people to the right intensity of care, place this level at Level 2.5, with at least 20 hours of clinical services a week. The Fourth Edition calls it high-intensity outpatient. The extra hours and closer clinical oversight are what separate PHP from IOP.

Most people come to PHP from one of two directions: stepping down after detox or residential care, or stepping up because weekly therapy or IOP is no longer enough.

Who PHP is for

PHP usually fits when:

  • You need more support than IOP but do not need round-the-clock monitoring.
  • You are medically stable, with no active withdrawal. If withdrawal is a risk, admissions can connect you with medically supervised detox at a network facility first.
  • You are leaving residential care and want a structured step down.
  • Your symptoms have worsened and you need close medication management and daily therapy.
  • You have a safe place to stay at night, at home or in sober housing.

PHP is not the right level of care if you need 24-hour medical supervision, inpatient detox or residential eating-disorder care. If you are unsure, call. Admissions will help you find the right level.

What PHP Treats

PHP programs in the network treat substance use and mental health conditions, often together:

  • Substance use: alcohol, opioid, stimulant, benzodiazepine and other drug use disorders, with relapse-prevention work while early recovery is most fragile.
  • Mental health: depression, anxiety, bipolar disorder, PTSD and trauma-related conditions, with daily therapy and medication follow-up.
  • Co-occurring conditions: a mental health condition and a substance use disorder treated by one coordinated team.

Care for Mental Health and Substance Use Together

For many people, the drinking or drug use and the depression, anxiety or trauma underneath it are one problem. Treating one half alone tends not to hold.

PHP is well suited to co-occurring care because the treatment day is long enough to hold therapy, group work and medication management in one place. The pairings network facilities see most often are anxiety, depression, PTSD and bipolar disorder, each alongside a substance use disorder.

  • Coordinated treatment planning. One team works from one plan for both conditions.
  • Therapy on both fronts. Individual, group and family sessions address the symptoms that drive use.
  • Medication management. Prescriptions are reviewed together so they work with each other, not against each other.

  • Cognitive behavioral therapy (CBT): a structured therapy that links thoughts, feelings and behaviors and teaches ways to change unhelpful patterns.
  • DBT-informed skills: emotion regulation, distress tolerance and relationships.
  • Motivational interviewing: conversations that build your own reasons and readiness to change.
  • Trauma-informed care, with trauma-focused treatment including EMDR: offered where clinically appropriate, to help process distressing memories so triggers carry less weight.
  • Relapse-prevention planning: a concrete plan for the high-risk situations you can already see coming.
  • Psychoeducation, mindfulness and wellness: practical skills for managing stress, symptoms and cravings.
  • Peer and recovery support, life-skills training, and crisis and safety planning.
  • Experiential and holistic therapies: creative, movement and nature-based work, such as adventure-based counseling and equine therapy at Chattanooga Recovery Center.

What to Expect, from Your First Call to Your First Week

  1. Call and check your benefits. One call reaches Recover Now admissions, who verify your insurance through a free, HIPAA-compliant check and tell you what your plan covers, your copay, and whether prior authorization applies.
  2. A placement assessment. A clinician talks with you about your history, current symptoms and prior treatment. The goal is the right level of care at the right facility.
  3. Intake at your facility. The facility's clinical team completes a full assessment, reviews your medications and builds a treatment plan with you.
  4. Your program starts. Day one covers the schedule, confidentiality, group guidelines and how to reach your care team.
  5. Your plan gets adjusted. Your care team reviews your progress and adjusts therapy, medications and level of care. Stepping down to IOP is a normal next step.

If your needs become more urgent during PHP, the facility's team coordinates a move to a higher level of care.

Most major insurers cover PHP when it is medically necessary. Your plan decides medical necessity, and most payers assess it using ASAM criteria. Prior authorization applies with some carriers.
